NYTD is a Kansas Department for Children and Families Information Technology Resource that must comply with Federal and State law. NYTD contains government information and may be accessed and used only as allowed by law for official DCF business and ONLY by authorized users. You will be provided an access code which allows you to complete the survey. It is important you do not share the access code with any other person.
Confidentiality of the information provided will be maintained to the greatest extent feasible. While there is no guaranteed right of privacy, all client/customer information is secured as required by law. Information in NYTD includes, but is not limited to, sensitive data encrypted to comply with confidentiality and privacy requirements.
All information or access to NYTD shall be used only for official purposes if authorized use has been violated. Unauthorized access or use of NYTD or information contained in such Database may result in disciplinary, civil and/or criminal action. I agree to abide by law, rules and regulations regarding the use and disclosure of individually identifiable protected health information (PHI) per the federal regulation 45 CFR Sec. 164.512(k)(6)(ii) and all other laws relating to Health Insurance Portability and Accountability Act (HIPAA). Access of use of NYTD information by any person constitutes consent to these terms.
